North Yorkshire County Council and it's partners including the National Health Service Trusts and Clinical Commissioning Groups are seeking to provide practical support to people who require such support on their return home.North Yorkshire County Council wishes to procure Home from Hospital Support to provide practical support for people leaving hospital to help them settle at home so that they are confident and competent to live independently.The Service will provide short-term practical support to people aged 18 or over leaving hospital to help them return home. The Service is intended to provide low level support to a large number of people which contributes to a whole system approach that is responsive to a person's individual needs.The key principles of the service are to:-- Assess and plan for short term support;-- Risk assess the home environment to determine all immediate basic needs; and-- Provide time limited practical support.Awards
