De SOJ richt zich op screening/triage en brede vraagverheldering (vanuit Positieve Gezondheid) van het gezin en omgeving. Daarnaast heeft de SOJ de mogelijkheid om voorliggend een aantal gelimiteerde (5-8) gesprekken te voeren. De SOJ heeft kennis van psycho-educatie, kan een gedegen advies geven. De gemeente Venlo zoekt naar een organisatie, die middels in te zetten medewerkers kan voorzien in de invulling van de functie, de rol en werkwijze van de SOJ voor de komende jaren binnen de gemeente Venlo. De organisatie (lees SOJ) werkt direct samen met de huisartsenpraktijken binnen de gemeente Venlo, de GGD/JGZ en het onderwijs in de gemeente Venlo. In de gemeente Venlo zijn er in totaal 15 huisartsenpraktijken. Een SOJ ondersteunt al deze praktijken bij hun hulp- of ondersteuningsvraagstukken.Awards
